Community Mental Health Center, Inc. is a provider of comprehensive mental health services, offering inpatient, outpatient, home-based, school, and community-based programs to individuals and families.
Position Responsibilities: The Transitioning Youth Facilitator engages youth or young adults in services while creating a crisis plan and stabilizing any crises. They explore with the youth/young adults their strengths and needs, their culture and visions. This position establishes an integrated care plan with them and monitors their progress toward goals and objectives. The Facilitator creates a transition plan including post-transition crisis management plans and celebrates successes with the youth/young adults.
Qualifications: Bachelor Degree in social work, counseling or related field; excellent organizational and communication skills; commitment to Transition to Independence Process (TIP) principles (Youth or Young Adult voice and choice, Strength-Based, Individualized, Outcome Based, Team Based, Culturally Competent, Community Based, Unconditional Care, Collaboration, and Natural Supports).
Consultation and collaboration with community systems to facilitate linkage, referral, and advocacy ensuring youth or young adults have access to the services and resources needed to reach goals and objectives supporting their vision.
Teach youth and young adults the skills needed to facilitate their team toward reaching their vision.
Assess strengths and underlying needs of youth and young adults.
Assess all TIP domains: employment/career, education, living situation, personal effectiveness/well-being, and community life functioning.
Teach youth and young adults how to identify underlying needs and utilize mitigating strengths to facilitate the development of an individualized plan that supports their family vision.
Monitor progress toward meeting established goals and adherence to action plans being sure that all domains are addressed as prioritized.
Assists in the development of natural supports and non-traditional community resources that are needed for a youth or young adult to reach their vision.
Administrative Supervisor: The administrative supervisor is the Program Manager of TIP within the Intensive Family Services Department.
